About the Game

Designed for restaurants, bars, event venues, and other businesses looking to verify customers’ vaccination status, the SMART Health Card Verifier App enables users to quickly verify an individual’s vaccination status. Scanning a SMART Health Card QR code reveals:

- Whether the SMART Health Card is valid (not tampered with)
- Whether the issuer is in The CommonTrust Network’s Registry of trusted issuers
- Key information on the SMART Health Card (issuer name, vaccine type, dates of vaccine doses, and name and date of birth of vaccine recipient)

The SMART Health Card Verifier App is a service brought to you by The Commons Project Foundation. It is built on standards from SMART Health IT and powered by Affinidi


NOTE: The Verifier app only scans SMART Health Cards. It does not scan paper CDC cards.

What is a SMART Health Card?

A SMART Health Card is a digital or printed version of your vaccination history or test results, shared through QR Codes and issued by supported states, pharmacies, and providers.
